Nitrofuran系化合物の使用経験(特にFuradantinによる尿路感染症治療を中心として)

Abstract

1) Chemical properties, antibacterial effects, blood and u r i nary concentrations, toxicities, and applications of nitrofuran derivatives, such as nitrofurazone (Furacin), G uanofuracin, Altafur, Furaspol, nifuroxime (Micofur), Furmethonol, furazolidone, and nitro f urantoin (Furadantin), being clinically employed have been presented. 2) Administration of furazolidone or Furadantin t o the patients with urinary tract infection has been attempted. When furazolido n e was administered to 7 patients with urinary tract infection, it was found that furazolidone was effective in 2 patients, fairly effective in 1 patient, futile in 3 patients, and unknown in 1 patient. Side effects along with administration of furazol i done, mainly gastric upset, were free in only 2 patients. However, it was found th a t slight side effect was found in 1 patient and moderate to severe side effect in 4 patients. There f ore, it is suggested that furazolidone produces poor therapeutic effect on urinary tract infection with high incidence of unpleasant side effects. When Furadantin was administered t o 19 patients with urinary tract infection, it was found that Furadantin was significantly effective in 3 patients, effective in 9 patients, fai r ly effective in 3 patients, futile in 1 patient, and unknown in 3 patients. It was observed that no side effect was found in 13 cases, slight side effect in 2 cases, and moderate to sev ere side effect in 4 cases. Furadantin possesses low toxicity and broad antimicrobial spe c trum with low incidence of appearance of resistant bacteria. It is also capable of a l o ng term administration of Furadantin and of easily reaching the effective urinary concentrat i on. Therefore, Furadantin is considered to be one of the satisfactorily effect i ve drugs for treatment of urinary tract infection. 3) According to the fa c t that nitrofuran derivatives, such as nitrofurazone etc., possesses antispermatogenetic action, the reports of administration of the drugs as a che m o therapeutic for testicular tumor were reviewed. The results of administration of nitrofurazone to 3 cases of testicular tumor were described. Unfortunately they were short term administrations. Therefore, it is necessary to obtain a large series with long term administration in order to get conclusion for effectiveness of nitrofurazone for testicular tumor. Furthermore, mechanism of action of nitrofurazone was discussed from a standpoint of histogenetic theory of testicular tumor.
