抄録: In the case of chemotherapy of the urinary tract infection, the influence of urinary pH upon drug effect was investigated. Children with Urinary tract infection were subjected for this study. Antibacterial effect of drug or on sensibility of drug for bacteria was studied by changing pH of the medium which is normally used for sensitivity test by means of minimal inhibitor concentration (MIC) for bacteria, and furthermore the influence of inoculated bacterial count was also studied. 1) In 1609 cases, the urinary pH ranged between 4.6 and 8.0 and the average was 5.94. Among these cases, the average urinary pH of patients with urinary tract infection was 5.89, while the average urinary pH of patients without urinary tract infection was 5.98. In the cases of Proteus infection, the average urinary pH was 6.8 in acute period and was 6.0 in recovery period, but the average over 7.0 was not noticed. The changes of the daily urinary pH of the patients of the urinary tract infection of children was within 1.1. 2) The influence of pH of the medium at the time of MIC measurement was studied by adjusting of pH at 5, 6, 7 or 8. Among the examined 41 strains except one, any bacterial growth was not observed with-out antibiotics if pH was adjusted at 5. Growth inhibitory effect by pH itself was noticed. 3) As to CEZ, ABPC, MPC and GM, we measured the MIC for E.coli, Proteus, Pseudomonas aeruginosa, Serratia marcescens, Staphylococcus aureus for each drug by changing pH of the medium and bacterial count. The result: a) The group in which pH of the medium increased antibacterial effect in alkaline side includes GM for various bacteria like E.coli., Proteus, Pseudomonas aeruginosa, Serratia marcescens, Staphylococcus aureus and MPC for Staphylococcus aureus. Among these, MPC for Staphylococcus aureus showed the contrary phenomenon to the previous concept that acid material usually decreases its antibacterial effect in alkaline side. The group in which antibacterial effect was increased in the acid side, includes CEZ and ABPC for E. coli, Proteus, Pseudomonas aeruginosa, and MPC for E. coli. The group in which effect was not influenced by pH, includes CEZ and ABPC for Serratia marc esc ens, Staphylococcus aureus and MPC for Proteus, Pseudomonas aeruginosa and Serratia marcescens. b) As to relationship between inoculated bacterial count and MIC, each of combinations such as CEZ for proteus, ABPC for E.coli, Proteus, MPC for E.coli, Proteus, Staphylococcus aureus and GM for E. coli, Proteus, Staphylococcus aeruginosa showed increased MIC along with increase of bacterial count. 抄録: The present paper reports the isolation and characterization of drug resistant E. coli from the urine from the hospital out-patients and in-patients from 1976 to 1978 December. E. coli strains isolated from urine speciemens during the above period were examinated for sensitivity against 12 drugs such as aminobenzilpenicillin, carbenicillin, minocycline, colistin, nalidixic acid, cephalothin, cephazolin, kanamycin, gentamicin, dibekacin, tobramycin and polymixin. From the urine of out-patients women with acute cystitis, 515 strains of pathogens were isolated during that period, and 388 strains (75.3%) of them were E. coli. From the urine of in-patients, 193 (8.9%) out of 2714 strains isolated from the urine were E. coli. The sensitivity pattern of the above drugs against E. coli from the urine of out-patients showed better sensitivity than that from in-patients. The difference of sensitivity for each drug was smaller in out-patients (from CB-PC 68.8 to GM 100%) than in in-patients (from AB-PC 21.1 to GM 98.4%). Among double drug resistance in out-patients and in-patients, the most frequent combination was AB-PC and CB-PC. 抄録: The fine structure of the transitional epithelium of adult dog ureter has been studied in thin sections with an electron microscope. It is thought that the light cells represent an active stage and that the dark cells represent a less active stage of the same cell type according to the shape of nucleus and cell organella. The dark cells are also consideced as a type to be the result of pressure by adjacent cells or newly formed cells. The free luminal surface of the celis and many cytoplasmic vesicles in these cells are bounded by a thick unit membrane. These vesicles or vacuoles are considered to function as a defense of the cytoplasm, when the epithelial cells change the form. The vesicles and the plasma membrane maintain angular conformations which suggest the membrane to be unsually rigid. It is supposed that the plasma membrane of the superficial cells is the major barrier to the free flow of water from the cytoplasma into the hypertonic urine. The possible origin of the thick plasma membrane in the Golgi apparatus was shown. All the cells of the epithelium have a dense feltwork of tonofilaments which ramify throughout the cytoplasm. The ton8filaments connect frequently with intercellular desmosomes. 抄録: The immune response in 55 uremic patients maintained on hemodialysis was measured with PPD, PHA skin test and PHA induced blastoid transformation of peripheral lymphocytes in in vitro system. Positive rate of PPD skin test was 62% in uremic patients, which was low in general Japanese adults. Mean diameter of erythema of skin reaction at 48 hours after the intradermal injection of 5 µg PHA was 21.1±11.8 (mean±SD) mm in uremic patients and 36.6±17.3 mm in normal control subjects. The difference between them was significant at P<0.001 Blastoid transformation of lymphocytes was determined with quantitation of tritium thymidine incorporation (cpm) and the ratio of blastogenasis was revealed as stimulation index (SI). S1 was 32.1±25.5 in uremic patients and 50.2±27.2 in control subjects. The difference between them was significant at P<O.OI. S1 was low in the uremic patients with high serum creatinine level. On this investigation, cellular immune response was reduced in uremic patients and lymphocyte transformation using whole blood culture was useful method for evaluation of cellular immune competence in severe anemic patients.1980-07-01T00:00:00Z
