小児に発生した陰嚢内血管腫の1例

Abstract

2歳6ヵ月男.検診で左陰嚢内の腫瘤を指摘され, 圧痛はなく, 超音波断層法で精巣, 精索, 精巣状態とは区別できる0.7×0.7cmのiso-hypoechoic lesionであった.悪性疾患ではないと判断して経過観察していたが, 陰嚢縫線下から左方に凹凸不整な小指頭大, 示指頭大の2個の腫瘤を認め, 超音波所見で1.5×1.0cmと0.5×0.5cmであった.median raphe cyst増大傾向を示す血管腫を疑い, 腫瘤摘出術を施行した.腫瘤は両側精巣とは容易に分離され, 2個の腫瘤は索状物でつながっていた.病理組織所見でcapillary-veinよりなる血管の拡張, 増生を認め, capillary and venous hemangiomaと診断した.術後経過良好で再発はない

The patient was a boy two years and six months old. He was brought to our University Hospital with the chief complaint of scrotal mass for two years. On palpation, the mass was discriminated from the testis, epididymis and spermatic cord. The mass was followed by ultrasonography and we found two masses at the age of two years and six months. The tumors were surgically resected and they were 0.5 x 0.5 x 0.5 cm, 1.5 x 1.0 x 0.5 cm in size, respectively. Histopathological examination revealed venous and capillary hemangioma. We discuss this case, and review the literature.
