膀胱腫瘍術前深達度診断の検討

Abstract

膀胱腫瘍における膀胱全摘および部分切除術患者28例が, 術前検査(CT, 動脈造影, 膀胱造影, 双手診)で診断された腫瘍の深達度と術後摘出標本の病理組織学的深達度とを比較検討した.1) CTの正診率は15例中3例, 20%であった.2)動脈造影による正診率は17例中5例, 29.4%であった.3)膀胱造影による正診率は12例中4例, 33.3%であった.4)双手診による正診率は17例中8例, 47.1%であった

Twenty-eight out of 90 patients with bladder tumor seen from 1979 to 1982 were examined by CT, pelvic angiography, cystography and bimanual digital palpation for staging diagnosis preoperatively. Twenty-two patients underwent total cystectomy, and the other 6 patients underwent partial cystectomy. Preoperative staging (T) and postoperative histopathological staging (pT) were compared to define the diagnostic accuracy. The accuracy rate with CT was 20.0%, that with pelvic angiography, 29.4%, that with cystography 33.3%, and that with bimanual digital palpation 47.1%. Digital palpation had a better diagnostic accuracy rather than other examinations.
