発熱・尿閉を主訴とした急性散在性脳脊髄炎の1例

Abstract

58歳男。感冒様症状で風邪薬を服用したが尿閉, 40℃の発熱をきたし緊急入院した。意識清明, 下腹部に膨隆, 導尿で500mlの尿流出を認めた。前立腺は2.5横指, 中心溝を触れ弾性硬であった。血清学的検査で単純ヘルペス等ウイルス感染を有意に示唆するものはなく急性前立腺を疑い, 抗生物質点滴, α1ブロッカーを投与した。入院時は自尿を認めたが3日目尿閉が出現し間欠導尿を開始した。熱は下降せず傾眠傾向, 下肢麻痺が出現し神経学的疾患の有無を検討した。感覚麻痺, 左右深部腱反射低下, 左優位の近位筋優位の脱力を認めた。MRIで脳髄質にガドリニウムにより造影されるT2強調画像で高信号像を示す病変を認めた。尾症候群による尿閉が考えられ髄液所見より髄液細胞数, 髄液蛋白増加を認めウイルス感染が疑われた。馬尾症候群の原因として急性散在性脳脊髄炎(ADEM)が強く疑われステロイドパルス療法を施行した。

Acute disseminated encephalomyelitis is an acute inflammatory demyelinating disorder of the cenrtral nervous system and patients usually present with signs of meningitis, depressed levels of consciousness, disorientation and/or motor or sensory deficits, depending on the affected sites. We present an unusual case of acute disseminated encephalomyelitis with the chief complaint of high-grade fever and urinary retention instead of any signs of neurological disturbances. This is the 10th report of this kind of presentation in Japan.
