Abstract

Judging from the literature available on the mor p hology of the prostate, I found that the authors of these literatures at home and abroad concentrated their attentions on prostatic hypertrophy and prostatic cancer, had numerous achievements in them, and investigated these matters to the minutest details. In recent years, on male hypogonadism its foundation has been made secure by the classification in the biopsy of testis and the estimation of gonadotropin. Therefore the papers on male hypogonadism have a tendency to put emphasis on testicular histology and neglect accessory sexual organs. Concerning seminal vesicles, one of the adnexa, Ishigami, Sakatoku, Urabe and Mori had records on its roentgenological views. But it may safely be said that there are few coherent literatures in studies of prostatic histology in hypogonadal male. As matters now stand, there are many results of animal experiments, but investigations are reticent about clinical cases. I have thought that for the reason that method s of the morphological study on the prostate are comparatively difficult, its study has not been yet. I have accomplished my object with the aid of prostatic needle biopsy. Prostatic biopsy has developed diff e rent methods out of the meaning of diangnostics on prostatic tumor, but each of these methods has some advantage as well as disadvantage. I have investigated these methods and practised perineal needle biopsy with Vim Silverman's needle. It is said that needle biopsy has a low fit-rate. But in my oun experiment, fit-rate is 82% in normal cases, 77%, in hypogonadal male. In this paper, I have also studies the literatures on male hypogonadism ; and then investigated embryology, anatomy and physiology on normal prostate. And I have described Saito's classification of histological type of prostatic gland. The same calssification has applied correspondingly to the following ; 1. normal type 2. complicat e d tubular type 3. adenomatous proliferou s type 4. cystic type 5. atrophic type and 6. deceptive cell p roliferation type. In the normal cases I underwent biopsy of 11 cases which have normal sexual ability and fertile, and succeeded in 9 cases. I have discr_bed histological finding of these 9 cases, 5 of type 1, 2 case of type 2, one case of type 3 and one case of type 4 respectively.
