腎阻血手術に対するCoenzyme Q10の応用

Abstract

Ten patients needing ischemic renal surgery for renal calculi were operated on at room telnperature after administration of Coenzyme Q10 (COQ10) which was used to protect the kidney involved. COQ10 was administered by intravenous drip infusion for 2 days before operation and on the day of operation in a dose of 1.5 mg/kg/day, and for 5 postoperative days in a dose of 0.5 mg/kg/day, 7.0 mg in all. During the period of treatment, blood levels of 24 to 64µg/ml of COQ10 were maintained. The ischemic times for the 10 patients ranged from 32 to 68 min, averaging 50 min. The postoperative course of all the patients was uneventful and without any undesired side effects from the treatment. Renal function was monitored by excretory urography before and after operation. Pelviocalyceal dilatation and delay in appearance of the contrast medium that were present in 7 patients before operation were improved in all of them after operation, and renal function aggravated in none of the patients. Judging from the increase in the serum level of lactic acid dehydrogenase, COQ10 was considered to have produced an effect tantamount to renal protection by surface cooling, and COQ10 seems to be of use in ischemic renal surgery lasting for up to about 60 min.
