泌尿器科領域におけるテトカイン(ポントカイン)による腰椎麻酔法

Abstract

In America and Europe Pontcain has been wi d ely used for lumbar anesthesia for these twenty years or so. Recently the same material as Pontcain was synthesized and sold as Tetocaine by Kyorin Pharmaceutical Company. We prepared 0.4% high specific gravity so l ution of Tetocaine by dissolving it in 6% glucose solution and administered this for lumbar anesthesia in urological operations. In neary all of the 37 cases rapid effect of anesthesia obtained and maximum duration of its effect was 30 minutes, minimum one hour 52 minutes, average 64 minutes. Nexed 0.1% low specific gravidity solution of Tetocaine prepared with distilled water was administered for continuous lumbar anesthesia. In the 4 cases the operations of 2 hours 11 minutes to 4 hours 35 minutes were acomplished without giving any pain to the patients by administering 10cc of the Tetocaine solution for initial dose and 2.5cc for additional dose from 1 to 4 times. Duration of the anesthetic effect by the initial dose was 1 hour 20 minutes to 2 hours, on the average 1 hour 40 minutes. Fluctuation of the blood pressure after lumbar anesthesia by T e t ocaine solution was small and no case of shock was demonstrated. In some case of nervous patients nausea and vomitting were observed but other remarcable side reaction was not observed.
