| dc.description.abstract | 73歳男.主訴は排尿時痛, 頻尿.DIPでは左尿管L5の高さに, 径1.5cm, onion skin様の陰影欠損を認めた.腹部CTではDIPの陰影欠損と一致する部位に石灰化像を伴わない, 径2.5cmの充実性腫瘤を認めた.画像より左尿管腫瘍と診断し, 後腹膜鏡下左腎尿管全摘除術を施行した.摘出標本では腎盂尿管移行部から約3cm膀胱側に長径約2cmの灰白色, 境界明瞭な弾性軟の腫瘍を認めた.腎腫瘍は黄色調で被膜により腎実質とは境界明瞭な脂肪成分に富む充実性の腫瘍であった.病理学的に, 尿管腫瘍弱拡大像では尿管内腔に向かって著しく肥厚した角化層を伴う扁平上皮を認めた.扁平上皮化生と正常粘膜との境界は明瞭であった.下層は軽度の炎症性細胞の浸潤を認めるのみで, 扁平上皮と下層との境界は明瞭であった.以上の病理所見より, 左尿管扁平上皮化生, 左腎血管筋脂肪腫と診断した | ja |
| dc.description.abstract | This paper reports a 73-year-old man who developed ureteral squamous metaplasia. Preoperative drip infusion pyelography showed a ureteral tumor with a major axis measuring 2.5 cm, which had a smooth surface and a broad base. Computed tomography findings suggested that the lesion might be a submucous mesodermal tumor or inverted papillary transitional cell carcinoma. Therefore, retroperitoneal laparoscopic left nephroureterectomy was performed. | en |
