| 抄録: | 85歳男.移行上皮癌と腺癌が混合した腎盂腫瘍の症例について報告した.左水腎症を主訴とし, 軽度の腎機能の低下が認められた.CTにて左腎盂尿管移行部から尿管にかけて造影効果を有する充実性の腫瘤性病変を認めた.左腎カテーテル尿細胞診はclass IIIbであった.左尿管腫瘍の診断にて, 左尿管全摘出術を施行した.腎盂尿管移行部に乳頭状の腫瘤を認め, 腫瘤の腎盂側(上側)では移行上皮が7-10層に重なり乳頭状増殖しており一部固有筋層まで及んでいた.腫瘤の尿管側(下側)では大小不同を示す上皮細胞が内腔を閉塞するとともに, 周囲脂肪組織に浸潤して血管侵襲を伴っていた.腎盂側はgrade 2の像を呈する移行上皮癌, 尿管側は中分化型腺癌である混合型腎盂癌と診断された An 85-year-old male was admitted to our hospital with the chief complaint of a left hydronephrosis. Computed tomography (CT) revealed left hydronephrosis and a left ureteral tumor. We performed left nephroureterectomy. Microscopically, the neoplasm was composed of a mixture of transitional cell carcinoma and adenocarcinoma. To our knowledge, this case is the 14th report of mixed carcinoma of the upper urinary tract. |
