| dc.description.abstract | 63歳男.53歳で右鼠径ヘルニア手術, 57歳時には腸閉塞となり, 保存的治療で軽快した.3年前に排尿障害があり, 治癒していたが, 今回, 頻尿, 排尿障害が再発し, IVPで右下部尿管狭窄による水腎症を指摘されて精査の為, 入院した.IVPで尿管狭窄, 水腎症と腎盂内腫瘤を認め, 膀胱後部腫瘍を考えて開腹した.腫瘤は膀胱頂部, 右後壁にあり, 弾性硬で, 回盲部に至る広範囲, 高度な癒着があった.病理学的に虫垂近くの穿孔, 急性慢性虫垂炎(陳旧性)で, 腎盂腔内には膿瘍が認められた.腫瘤, 癒着腸管を切除して9ヵ月後のIVPでは水腎症が消失していた.典型的症状のない虫垂炎の診断は困難であるが, 本例は腫瘤切除を要した稀な例である | ja |
| dc.description.abstract | A case of right hydronephrosis as a complication of appendicitis is reported. The patient was a 63-year-old male, whose chief complaints were pollakisuria and dysuria. Roentgenographic examination revealed right hydronephrosis due to stenosis of right lower ureter and intrapelvic tumor at the right side. Exploration revealed an abscess in the pelvic cavity, and pathologic examination disclosed periappendiceal perforation and an old inflamed appendix. A review of the literature revealed that it is difficult to diagnose the appendicitis without typical symptoms. | en |
