糖尿病性神経因性膀胱と心電図R-R間隔変動係数について

Abstract

糖尿病患者12名にCV値測定と尿水力学的検査を施行した.CV値2%以下の症例では残尿量も多く, 膀胱容量も拡大し腎機能障害や視力障害などの合併症を有していた.R-R間隔変動係数は膀胱機能と密接に関連し, 膀胱症状のない症例においても定期的に本検査法を施行することにより, 膀胱機能障害を早期に発見し, これら症例に精密な尿水力学的検査を施行することにより, 適切な尿路管理を行うことも可能である

Urodynamic investigations and the measurement of the R-R interval on ECG have been applied to assess the extent of autonomic and peripheral neuropathy in 12 patients with diabetes mellitus. Objective evidence of neuropathic bladder dysfunction was detected in 6 patients (50%) and these cases were included under 2% of the coefficient of R-R interval variation. The commonest abnormality was hypotonic, large bladder and marked residual urine, which was usually asymptomatic. It is concluded that in diabetic patients this simple autonomic function test is a good guide to vesical function.
