泌尿性器系重複悪性腫瘍の文献的・統計的考察 --附: 同時性診断・同時治療をなしえた重複癌の1例(膀胱と直腸)--

Abstract

1)本邦泌尿性器系重複腫瘍541例につき文献的統計的考察をおこなった.2)男性が83.1%を占め, 平均年齢は男性66.6歳, 女性63.2歳であった.3)泌尿性器系悪性腫瘍は膀胱が49.7%でもっとも多く, ついで腎24.8%, 前立腺23.2%の順であった.4)重複腫瘍の組み合わせでは消化器系悪性腫瘍との組み合わせがもっとも多く, 泌尿器系同志の組み合わせのほか, 前立腺と膀胱・肺, 膀胱と子宮・乳腺との組み合わせが注目された.5)平均発生(発見)間隔は4.3年であり, 最近は臨床診断される症例が多くなってきている.真性重複腫瘍とも言うべき同時性重複腫瘍は54.8%を占めた.6)予後は悲観的であった.7)前立腺をはじめとして泌尿性器系重複腫瘍の発生頻度は高く, 全重複腫瘍の10%強を占める

A review of the literature was made on multiple primary malignant neoplasms associded with genitourinary organs. A statistical study was also made on 541 such cases (including our case) previously reported in Japan. The average patient age was 66.0 years old, and 83.1% of the patients were men. The genitourinary malignant neoplasms were reported most frequently in the bladder (49.5%), kidney (24.8%) and prostate (22.8%). The other organs most frequently having neoplasms accompanying the genitourinary malignant neoplasms were the stomach (25.5%), lung (11.2%) and large intestine (8.4%). The average interval between the first and second primary malignant neoplasms was 4.4 years; 54.8% were synchronous multiple primary malignant neoplasms. A case of a 73-year-old man with synchronous multiple primary malignant neoplasms, i.e. transitional cell carcinoma of the bladder associated with adenocarcinoma of the recto-sigmoidal portion, is also reported.
