陰茎の原発性細網肉腫の1例ならびに原発性陰茎肉腫の文献的考察

Abstract

1. A case of reticulum cell sarcoma manifested primarily in the corpus cavernosum of the penis is reported. A 70-year-old man was admitted to our hospital with chief complaint of a painless firm mass of the shaft of the penis proximal to the glans penis of one month duration. The partial penectomy was performed with postoperative chemotherapy with 5 FU (250 mg×30) and tele-Co60 irradiation (42407 each side) to the both inguinal region. The histological examination revealed reticulum cell sarcoma in corpus cavernosum penis. Unfortunately, 4 months after first operation the recurrence of this tumor was noticed at the root of the penis and the patient complained of a perineal mass and slight dysuria. As the second operation, total penectomy with perineal urethrostomy was done, and postoperative chemotherapy with 5 FU (250mg×30) and Cosmegen (1mg×3) and tele-CO6o irradiation (41167) to the region of penile root were performed again. This patient is alive without any clinical evidence of recurrence or metastasis 13 months after first operation. 2. Authors could collect only 3 cases of primary reticulum cell sarcoma of the penis including authors' case reported only in Japan and not in Europe and U. S. A. up to date, and 3 cases of secondary reticulum cell sarcoma of the penis reported in Japan and U. S. A. up to date, and statistical survey was made on these 6 cases of penile reticulum cell sarcoma and discussions were added on its clinical features, diagnosis, pathohistology, therapy, prognosis and metastasis. 3. Authors summarized 25 cases of primary penile sarcomata exclusive of melanoma from the literature up to date including 3 cases of primary reticulum cell sarcoma. A statistical survey was made on 107 cases of penile primary sarcomata (72 cases from Abeshouse, B. S. et al. 1962, 10 cases from Sugita, A. et al. 1963, and 25 cases from authors, 1973). Discussions were made on its clinical features with brief consideration of priapism due to primary or secondary tumor of the penis, diagnosis, therapy, prognosis, metastasis, local recurrence and predisposing factors.
