気腫性腎盂腎炎の1例

Abstract

This report describes the first case of emphysematous pyelonephritis in Japan, which is characterized by systemic toxicity, necrosis of renal parenchyma and formation of gas in and around the kidney. A 55-year-old woman was admitted to the hospital with severe diabetes, persistent high fever with chills, and pain and tenderness in the left flank in spite of the therapy by antibiotics and drip infusion in another hospital. DIP revealed non-visualization of left kidney and specific perinephric gas bubbles. Her general condition got worse and a palpaple tender mass in the left loin became larger. After incision and drainage, she had become afebrile and regained strength with good control of diabetes. But recurrence of pyelitic fever attack and no improvement of left renal function made us decide to remove the left kidney. Following nephrectomy she recovered promptly. Many foreign previous cases were reviewed.
