非特異性肉芽腫性前立腺炎(non-specific granulomatous prostatitis)の1例

Abstract

A 73-year-old man was admitted to our hospital because of the chief complaint of abrupt onset of urinary retention on April 20, 1978, There was neither history of allergic disease nor tuberculosis. Digital examination of the prostate per anum revealed an egg-sized, slightly hard, irregular surfaced mass. A few nodules were palpated over the right lobe. A specimen of the prostate obtained by perineal needle biopsy was diagnosed histopathologically as chrcnic prostatitis without malignancy. The levels of serum acid phosphatase and prostatic acid phosphatase were within normal limits. A cystourethrogram showed moderately prolonged posterior urethra and elevated bladder floor. On May 26, suprapubic simple prostatectomy was performed under the diagnosis of benign prostatic hypertrophy. Enucleation of the adenoma was difficult due to marked adhesion to the surgical capsule. The weight of the prostate mass was 20 g. The histopathological diagnosis was made as non-specific granulomatous prostatitis. Postoperative course was uneventful. Including our case, 6 cases of non-spesific granulomatous prostatitis have been reported in Japanese literatures. The diagnostic, therapeutic and prognostic problems were discussed in this paper.
