膀胱癌の血管構築に関する臨床的研究(1) -Pharmacoangiographyを併用した選択的内腸骨動脈造影による膀胱癌の浸潤度の評価-

Abstract

Pelvic angiography, particularly, bilateral selective hypogastric arteriography has been used for the assessment of the clinical staging of the bladder tumor preoperatively_ To define the stage of the tumor infiltrations, it is necessary to evaluate vascular constructional patterns and characteristic features in the vesical wall. For this purpose, pharmacoangiography using vasoconstrictive agents such as adrenalin or angiotensin II was combined with a regular angiographic technique and the angiographic stagings were compared with those diagnosed clinically and histologically. In general, there was a good agreement between angiographic stagings and clinical stagings in 21 cases but a more accurate correlation was observed between findings of pharmacoangiography and clinical stagings. Thus, the diagnostic accuracy for the tumor staging increased to 85.7% in this method from 60.0% in the conventional pelvic angiography.
