小児における膀胱および前立腺横紋筋肉腫の治療経験

Abstract

Four children with vesical or prostatic embryonal rhabdomyosarcoma, 3 patients (I girl and 2 boys) in the bladder and one patient in the prostate, were seen at our department during 6 years from 1973 to 1978. The first patient (4-year-old girl) with rhabdomyosarcoma of the bladder of stage Ia is free of tumor 64 months after total cystourethrectomy followed by a combination chemotherapy of actinomycin D and vincristine. The second patient (I-year-old boy) with rhabdomyosarcoma of the prostate of stage III received total cystoprostatectomy, ileal conduit, a combination chemotherapy of actinomycin D and vincristine and postoperative irradiation 5, 500 rads. A recurrence of the tumor appeared in the inguinal region II months later. An additional treatment of irradiation and chemotherapy was performed and the boy died of heart failure 5 months later. The third patient (14-year-old boy) with rhabdomyosarcoma of the bladder of stage IV received a combination chemotherapy of adriamycin, vincristine and cyclophosphamide prior and posterior to surgical intervention, and postoperative irradiation 2, 000 rads. He is free of tumor 19 months after initial chemotherapy. The fourth patient (3-year-old boy) with rhabdomyosarcoma of the bladder of stage IV was treated only by a combined chemotherapy of adriamycin, vincristine and cyclophosphamide. A partial remission was obtained and he is alive 10 months after chemotherapy. The usefulness of preoperative combination chemotherapy was discussed.
