膀胱頚部硬化症の内視鏡診断

Abstract

In order to clarify the endoscopic characteristics of bladder neck contracture (BNC), 17 confirmed cases were selected and the findings ofurethrocystoscopy (UCS) were compared to those of 12 patients with small BPH. The UCS findings were categorized as 4 grades from normal to severe change, with score points from 1 to 4, in 4 aspects such as distance from internal urethral orifice to the upper margin of verumontanum (PU), elasticity of the bladder neck, the form of internal urethral meatus and the grade of vesical trabeculation (Tr). The statistical analysis of the two groups revealed that BNC has significantly shorter PU (p<0.01) and possibly severer Tr (p<0.01) than BPH. The PU of ENG is not longer than 1.3cm and that of BPH is not shorter than 2.0 cm (p<0.05, respectively). The 95% confidential limit of the difference of both PU is approximately 0.4 to 1 cm. This significant shortness of prostatic urethra of BNC may be related to the etiology of the pathological condition, that is periurethral musculature may also be involved as well as bladder neck.
