三角筋への遠隔転移を伴った膀胱癌剖検例

Abstract

A 65-year-old man, who had already underwent bilateral cutaneous ureterostomy for bilateral hydronephrosis due to bladder tumor at the other hospital, was referred to our hospital because of general malaise and high fever on December 19, 1979. In spite of forceful administration of antibiotics and immunoglobulin, he died from sepsis on January 7, 1980. Postmortem study revealed metastases to retroperitoneal lymph nodes, diaphragm, pleura, lung, adrenal gland, liver, vertebra, rib and deltoid muscle. It is a well-known fact that metastatic carcinoma is very rare in striated muscle. An autopsy survey of the bladder tumor was made on 1105 cases recorded in the Annal of the Pathological Autopsy Cases in Japan during the past 5 years from 1973 to 1977. Incidence of metastasis was 70.4% and sites of metastasis were listed. Metastasis to muscles were recorded in 14 cases; these were ilio-psoas muscle, internal oblique muscle and intercostal muscle. The deltoid muscle was not reported as a site of metastasis in this survey. Several literatures concerning about secondary tumor in muscle were reviewed and some discussion was done.
