抄録: | Simultaneous measurement of cystometry and electromyography of external urethral sphincter was performed on sixty five cases (54 male, II female), and the comparative studies of detrusor pressure to total vesical pressure was done in the cystometric examination to make diagnosis of detrusor-sphincter dyssynergia, and the following results were obtained. 1. The curves like detrusor reflex due to the abdominal pressure were found in 4.6 per cent of the cases. 2. Because of physiological contraction of the external urethral sphincter caused by straining, 3.1 per cent of cases had a p8ssibility of misdiagnosis as detrusor-sphincter dyssynergia. 3. The possibility of psychic inhibition was considered between 9.2-23.1 per cent of the cases with detrusor areflexia. 4. Abdominal pressure was not considered as an effective pressure for micturition. 5. The speed of instillation of carbon dioxide into the bladder gave no influence on the intravesical pressure. 6. Abnormal waves caused by intestinal peristalsis were revealed in around 10 per cent of the cases. 7. The artifacts of electromyograms due to the use of ring electrode were able to differentiate from detrusor-sphincter dyssynergia by the form of wave. From the findings as described above, it is considered that subtracted bladder pressure should be used in the study of detrusor-sphincter dyssynergia. |
