The subjective and urodynamic effects of an alpha adrenergic agent on voiding difficulty were studied in 21 patients with benign prostatic hyperplasia and 6 patients with atonic type of neurogenic vesical dysfunction. All patients were treated by oral administration of phenoxybenzamine 10 mg daily for 14 days. The effectiveness was determined by the degree of relief of symptoms related to micturition, and the improvement in rate of residual urine, maximum flow rate and urethral pressure profile. In benign prostatic hyperplasia, the subjective and objective symptoms were improved in 76.1 % and 73.6% of the patients, respectively. Particularly, the improvement in maximum flow rate was statistically significant compared to the value before treatment. In neurogenic vesical dysfunction, the subjective and objective symptoms were improved in 50% and 100% of the patients, respectively. From these results, phenoxybenzamine was considered to be useful in easing the voiding difficulty of patients suffering from benign prostatic hyperplasia and neurogenic vesical dysfunction.en
