Simple renal cysts are usually asymptomatic and produce no harm to the kidney. However, there are some cases which require treatment to eliminate calyceal obstruction. Therefore, since 1980, we have been performing percutaneous cystic puncture guided by ultrasonography. The cyst fluid was aspirated, and a water soluble contrast medium was instilled into it. Seven male and two female patients with a mean age of 64 years ranging from 47 to 80 years had cysts. Except for one case in which the cyst was hemorrhagic, eight cysts were followed by percutaneous puncture. Among them five cysts were found to have decreased in size. These cases were followed up from two to twenty months. Although the number presented is small and the length of follow-up in some cases is rather short, good therapeutic results may justify cyst puncture. We think that puncture should be the primary management and that most patients can be spared the greater hazards of surgical intervention.en
