Abstract: Herein we report a rare case showing spontaneous extrarenal escape of multiple renal stones. This case, a 34-year-old female, had been followed up for a distal type of renal tubular acidosis complicated with multiple renal stones, and also associated with Sjogren syndrome. The patient was admitted for spiked temperature caused by pyonephrosis due to ureteral obstruction by the stone. X-ray examinations revealed multiple stones at the extrarenal position. At nephrostomy and ureterolithotomy, neither perinephric abscess nor urinary infiltration were found. The multiple stones were suspected to have escaped from the kidney by calyceal perforation through the renal sinus, but not by perforation of the renal parenchyma or renal pelvis.
