cis-Diamminedichloroplatinumによる尿路性器悪性腫瘍の化学療法

Abstract

Eleven patients with advanced genitourinary cancers were treated with cis-diamminedichloroplatinum alone or in combination with other drugs or hormonal therapy or radiotherapy (4 patients with advanced nonseminomatous testicular tumors, two with recurrent bladder tumors, three with prostatic cancers, one with urachal tumor, one with liposarcoma of the spermatic cord). All of three patients with nonsemitomatous testicular tumors who were treated with cis-platinum, bleomycin and vinblastine showed complete remission and are alive and free of disease from 11 to 27 months. One patient with nonseminomatous testicular tumor previously treated with cis-platinum, bleomycin and vinblastine was treated with cis-platinum and radiotherapy, and he obtained partial remission and alive with residual disease for ten months. No objective response was seen in two recurrent bladder tumors. Three patients with prostatic cancers were treated with orchiectomy and cisplatinum, and all of them showed no response. No response was seen in one urachal tumors. Toxicity was similar to that previously reported, with gastromtestinal side effects (nausea and vomiting) and nephrotoxicity most commonly seen. Severe myelosuppression occurred in four patients treated with cis-platinum, bleomycin and vinblastine, but no patients were died of sepsis. We believe that the combination chemotherapy with cis-platinum appears to be active in disseminated testicular tumors but to be not so effective in other genitourinary cancers.
