<研究ノート>欧米社会における「ホスピスの医療化」研究の動向と展望

Abstract

本稿の目的は、欧米社会における「ホスピスの医療化」を扱った先行研究において「ホスピスの医療化」がどのように理解されているかを考察し、日本の「ホスピスの医療化」の実態を探求するにあたっての示唆を導き出すことである。社会学の「医療化」概念をもとに「ホスピスの医療化」を扱った先行研究を「専門化」、「制度化」、「商業化」に分類し、「ホスピスの医療化」がどのように理解されているかを考察した。その結果、研究が不十分な「商業化」については確認できないものもあるが、「専門化」と「制度化」による「ホスピスの医療化」を扱った研究では、ホスピスの理念の変容や、それによって導き出される否定的帰結および肯定的帰結の内容が示されていることが確認された。日本の「ホスピスの医療化」の実態を明らかにした実証研究はないため、これら先行研究から導き出される示唆を生かした研究による解明が望まれる。

This paper examines how previous Western research understands the medicalization of hospices, to derive implications for understanding the medicalization of hospices in Japan． I classify hospice medicalization into the three processes of specialization, institutionalization, and commercialization, and analyze their characteristics and consequences. Research on commercialization remains inadequate, but research on specialization and institutionalization shows changes in hospice ideals and their negative and positive consequences. Negative aspects of medicalization include the danger of subordinating the ideals of hospice to the aims of each specific process, whereas positive aspects include the use of medical processes to further the ideals of hospice. The lack of empirical research on medicalization of hospices in Japan suggests a need for further investigation of this topic.
