精神科作業療法とクライシス介入訪問 --被害妄想を持つ分裂病患者の場合--

Abstract

It has only been during the past 10 or 20 years that occupational therapy was gradually introduced into modern clinical psychiatry in Japan. However, due to the prevalence of inpatient modalities in Japanese psychiatric treatment, it has generally been done with long-term schizophrenic inpatients. Recently, community-based care has attracted significant interest, with occupational therapy focussing on the quality of life rather than merely the activities of daily living. With respect to social support, occupational therapy might play many roles, particularly within the method of "visiting as crisis intervention." Recently, we employed visiting as a method of crisis intervention with a schizophrenic outpatient who had delusions of injury. In this paper, we discuss the following five dimensions: 1. Visiting as crisis intervention 2. Plural visitors and a key person 3. Using visiting along with occupational therapy 4. Methods of visiting as crisis intervention 5. The relationship between visiting and occupation therapy Although many problems remain unsolved and visiting is not yet generally used, its achievements have attracted great attention.
