術後痛およびそのケアの患者の受け止め方とそれに対する看護婦の認識

Abstract

The patients who receive operation suffer from various stresses around operation. Pain which disturbs their safety and comfort is the major problem after operation. Recently, it has been generally accepted that aggressive pain relief has great benefits not only on the recovery of patients but also on their quality of life. In this study, 90 patients who received major surgery and 133 nurses who were working in surgical ward were surveyed by questionnaires regarding pain after operation experienced by patients and care for it by nurses, and the following results were obtained. 1. The most potent measure for pain relief was analgesics, followed by physical supports (positioning, massage, fomentation, etc.) and mental supports (attendance, explanation, chat, etc.). 2. The levels of pain where the patients wanted analgesics were lower than those where the nurses gave them to the patients. 3. The levels of pain the patients experienced were lower than those nurses assessed. 4. Most of the patients were satisfied of the care for pain relief by doctors and nurses. 5. One forth of the patients wanted more sufficient pain relief, indicating that there were still some patients who suffered from pain. Although the nurses recognized that the patients had strong pain after operation, half of the nurses thought that pain relief was enough for the patients. These results suggest that more precise assessment of pain and more appropriate care including physical and mental supports are necessary for pain relief after operation.
