人工股関節全置換術 (THR) 術後の股関節脱臼について

Abstract

After total hip arthroplasty, instability in the form of dislocation is second in frequency to loosening as a major complication of this procedure and may necessitate prolonged hospitalization or additional surgical treatment. The average incidence of dislocation is approximately 3%. Several factors may contribute to instability in THR. The important factors are as follows: 1) a history of previous hip surgery or revision total hip, 2) a posterior surgical approach, 3) faulty position of one or both components, 4) impingement of the femur on bone or cement, 5) impingement of the neck of the femoral component on the margin of the socket, 6) shortening of the femur, 7) insufficient or weak abductor muscles, 8) avulsion or nonunion of the greater trochanter. We think that prevention is the best treatment for THR dislocation. The motion precautions for the patients who have had a THR are described.
