<原著>びまん性汎細気管支炎症例のメサコリンに対する気管過敏性に関する検討 : 健常人, 気管支喘息患者および肺気腫患者との比較

Abstract

By use of Astograph, deviced by Takishima, we studied the bronchial responsiveness (sensitivity and reactivity) to inhaled methacholine in 8 patients with diffuse panbronchiolitis (DPB) and compared it to that in 20 patients with bronchial asthma, 2 patients with emphysema, and 17 healthy volunteers. Additional 3 patients with DPB, 3 patients with bronchial asthma, and 2 patients with emphysema were served only for measuring the control respiratory resistance (Rrs. cont.) during inhaling physiological saline. Positive responses to methacholine occurred in 7(87.5%) of 8 patients with DPB, 19(95%) of 20 patients with bronchial asthma, 2(100%) of 2 patients with emphysema, and 9(52.9%) of 17 healthy volunteers. The value of Rrs. cont. in the patients with DPB was significantly higher than that in the patients with bronchial asthma and in the healthy volunteers. Dose-response curves and time course to methacholine challenge differed in the patients with DPB, the patients with bronchial asthma, and the healthy volunteers, as follows. Respiratory sensitivity (Cmin and PC50) in the patients with DPB was as sensitive as that in the patients with bronchial asthma. Respiratory reactivity (St at PC50) of the patients with DPB was, however, as low as that in the healthy volunteers. Bronchial hyperreactivity is thus common in DPB but is different from that in bronchial asthma.
