| dc.description.abstract | 17歳男。右背部痛を主訴に受診した。超音波検査で1.6cm大の腫瘍性病変を認め、膀胱鏡では右尿管口外側に有茎性乳頭状腫瘍を認めた。排泄性腎盂造影(IVP)では上部尿路に異常はなく、膀胱部に陰影欠損は見られなかった。表在性膀胱腫瘍の診断で、経尿道的膀胱腫瘍切除(TUR-Bt)を実施した。術後経過は良好で再発予防のため塩酸ピラルビシン30mgの膀胱内注入療法を週2回、計8回実施し、術後約1年経過し再発は認めていない。本症例はIVP陰性であったものの超音波検査で15mm大の腫瘤影を認めた。右尿管口部に一致しドップラー法で尿管瘤からの尿の流出像共所見が取れたため、膀胱鏡を一旦躊躇したが、念のため膀胱鏡検査を実施したことが早期発見に繋がった。腰痛との関連は無いと考えられ、スクリーニングで実施された超音波検査が有用であったと考えられた。 | ja |
| dc.description.abstract | A 17-year-old male was referred to our hospital with the chief complaint of right back pain. Cystoscopic examination revealed a papillary tumor on the posterior wall following detection on screening ultrasound examination revealed a tumor in the bladder. Transurethral resection of the bladder tumor was performed. Histological examination of the excised tumor revealed transitional cell carcinoma, grade 1, pTa. No recurrence has been observed for about 1 year postoperatively. | en |
