| 抄録: | 64歳男性。患者は頻尿を主訴とした。精査の結果, 前立腺癌T4N0M0と診断され, MAB療法を開始したが, PSAの上昇, ふらつき, 手の震え, 呂律困難が出現した。髄液検査, 脳CT・MRIでは異常を認めなかったが, 各種腫瘍マーカーではPSA, CEA, NSE, s-IL2-Rの著明な上昇を認めた。前立腺再生検では, 免疫染色にてCEA, CgA, synaptophysin, CD56が陽性を示し, Neuroendocrine differentiationと診断, 骨盤内リンパ節転移に対し放射線照射を行い, PSAは低下したが, その後, 脳幹, 眼窩, 舌根部, 皮膚に転移を認め, 全身状態が悪化し, 患者は最終的に呼吸不全にて死亡となった。 Prostatic neuroendocrine (NE) carcinoma is a rare disease and the NE differentiation in prostate cancer is characterized by poor prognosis, rapidly progressing tumor and an androgen-independent state, for which there is currently no successful therapy. Herein, we report a case of NE differentiatiated prostatic cancer, which metastasized to the base of tongue, intraorbit and brain stem. This is the first Japanese report of prostate cancer metastasis to the brain stem and base of the tongue. |
