| dc.description.abstract | 67歳男。患者は58歳時に膀胱癌に対し膀胱全摘除術, Hautmann法による回腸利用新膀胱造設術を受けた。9年後の今回, 近医で骨折の精査中に膀胱結石を指摘され, 画像所見では新膀胱内に8×7.5cm大の, 何重にも層状になった巨大な結石が認められた。Lithoclastおよび砕石鉗子で経尿道的膀胱砕石術を施行したところ, 術後のIVPでは結石等の所見はなく, 通過も良好で, 以後は水腎症も認められていない。尚, 本症例は問診により初回手術初期は下腹部膨満感で排尿が行われていたが, 時がたつにつれて時間排尿に代わり, 更に時間排尿は年々ルーズになっていたことが明らかになった。このため徐々に残尿量が増加し, 慢性的な残尿の存在が結石形成の誘因となったと考えられた。一方, 術半年後のpressure-flow studyでは膀胱容量が100mlと小さく, その原因として長期間結石が放置されたことで, 新膀胱が機械的刺激を受けて萎縮したものと推測された。 | ja |
