パキスタン高所住民における胃十ー二指腸病変

Abstract

パキスタン, フンザ地域に在住の高所住民104名を対象に胃内視鏡検査を行い, 高所環境下における胃十二指腸病変の検討を行った. 男性42名, 女性62名, 平均年令41歳(13-75歳)0104名中29名(28%)に胃十二指腸病変をみとめた. このうち消化性潰蕩は5名(胃潰瘍2名, 十二指腸潰瘍3名)にみとめたが急性胃粘膜病変はみとめなかった. また日本人高齢者に通常認められる萎縮性胃炎はほとんど認めなかった. 以上により 1)高所住民には胃十二指腸病変が好発する. 2)病変のうち慢性胃炎が主である. 3)胃粘膜の自然経過が日本人と異なるなどの知見をえた.

We endoscopically examined the gastro-duodenal mucosa of Pakistani highlanders in Hunza. One hundred and four highlanders served as subjects. The mean age of 42 male and 62 female was 41.4 years old (range: 13-75 ). Twenty nine (28%) subjects had some gastro-duodenal lesions, including 5 peptic ulcers. But there were no acute gastric muosal lesions. Moreover, atrophic gastritis, which are common among old Japanese men, were very few. These results indicate as follows. l)A high incidence of gastro-duodenal lesions was observed in Pakistani highlanders. 2)Chronic gastritis• was common. 3) Natural course of the gastric mucosal cells in Pakistani highlanders are different from that in Japanese.
