精神分析的心理療法における行き詰まりと治療機序に関する実証的研究－スキゾイドパーソナリティの場合－

Abstract

本研究はスキゾイド的なクライエントとの精神分析的心理療法における行き詰まりと治療機序について実証的に探索したものである。15名の事例報告者から得られた言語データを修正版グラウンデッドセオリーアプローチによって分析し，6つのカテゴリーと25の概念が生成された。そして，その結果をもとに臨床プロセスが明らかにした。そのなかでセラピストが介入すべきか介入を控えるべきかというジレンマにもとづく「交流をめぐるアンビヴァレンス」を抱えることの重要性が示され，それは受身性と能動性を共存させる治療姿勢であることが明確にされた。そして，最後に精神分析的心理療法がパーソナリティのタイプに関わらず，すべてに共通する治療機序を備えているのか，あるいは特にスキゾイド的なクライエントに治療的反応を示しやすいのかという二種の新たな問いが生成された。This article empirically clarifies impasse and therapeutic mechanisms in psychoanalytical psychotherapy with schizoid personality patients. The language data of 15 case reports were analyzed by the Modified Grounded Theory Approach. The results yielded six categories and 25 concepts. The author clarified the clinical process of the psychoanalytical psychotherapy with schizoid personality patients. The analysis indicated the importance of holding “ambivalence on communication” based on the dilemma of whether the therapist should intervene or refrain from intervention. It was demonstrated that the therapeutic attitude allows the coexistence of passivity and activity. Finally, And, questions were raised regarding whether psychoanalytical psychotherapy provides a therapeutic mechanism common to all despite personality type or it is easy to show the therapeutic reaction to schizoid personality patients in particular.
