心理臨床家養成課程でクライエントの自殺に関する準備教育を行う意義 --精神科研修医を対象にした実践報告の概観をふまえて--

Abstract

担当患者やクライエントが自殺で亡くなるという出来事は精神科医や心理臨床家に深刻な影響を及ぼすため, 養成課程で準備教育を行うことの重要性が提案されている。本稿ではまず精神科研修医を対象としたそのような準備教育の実践報告を概観し, その知見をふまえ, 心理臨床家養成課程でクライエントの自殺に関する準備教育を行うことの意義を論じた。具体的には, このトラウマ的な体験に対処するのに必要な相互支援の促進につながること, 心理臨床家であることの本質を考える機会となること, そして心理臨床家が死に向き合う風土醸成の萌芽となる可能性の3 点について論じた。最後に, 自殺ハイリスク者への危機介入に関する教育においても, 介入に必要な知識や技能の習得だけではなく, クライエントの死の可能性と対峙し続ける心理臨床家としての姿勢を育むことが重要であり, その点でもクライエントの自殺から学ぶことは不可欠であることにも触れた。

The suicide of a patient or client has a profound influence on psychiatrists and psychotherapists, and it has been suggested to be important to provide preparatory education on patient/client suicide in training courses. This paper first presents a review of practical reports of preparatory education conducted for psychiatric trainees. Based on the findings, the significance of preparatory education on client suicide in psychotherapist training courses is discussed. Such training can promote mutual support among trainees to deal with traumatic experiences, and can provide opportunities to think about the essence of being a psychotherapist. In addition, this can lead to a new cultural climate among psychotherapists, which will enable them to confront death. Based on those discussions, education on addressing suicidal clients is also discussed. It is important not only to acquire knowledge and skills, but also to nurture a psychotherapist's attitudes to confront the possibility of the client's death. Learning from client suicide is essential for psychotherapists.
