精神科グループホームという<場>に関する臨床心理学的考察 --個別心理療法・精神科デイケアとの比較検討から--

Abstract

本稿は精神科グループホームが臨床心理学的にいかなる<場>であり, そこでの支援にはどのような特徴と可能性があるかについて論じるものである。グループホームは精神障害者が地域で暮らすための<場>であり, 居室という"私"的な"個"の空間だけでなく, 共有空間という"公"的な"集団"の空間にもなりうる"あわいの<場>"も内包されているという特徴を有している。構造が明確な個別心理療法と比べると, 暮らしの<場>であるグループホームは非常に緩やかな構造であり, その時々によって<場>の体験のされ方は流動的に変化する。また, デイケアにおける集団活動との共通点は多いものの, "個"の<場>が保障されているという点で大きな差異がある。地域での臨床実践においては個別心理療法のような枠組みでの支援が難しい場合も多いが, その都度その<場>全体を見立てながら個々の状況に応じた支援を組み立ててゆくことが必要であると考えられる。

This study discussed what kind of psychological "place" a group home for people with mental disabilities is and what features and possibilities there are to provide support in a group home. A group home is a home for people with mental disabilities within the community that has not only "private" rooms but also "public" spaces, which have the potential to be "group" places. The boundaries between "private" rooms and "public" spaces are easily accessible and the construction of a group home is loose compared to individual psychotherapy with a clear structure. Therefore, the experience of "place" changes in a fluid way depending on the situation. Although there are many similarities with group activities in psychiatric day care, there is a large difference in that "private" rooms are guaranteed in a group home. In clinical practice in the community, it is often difficult to provide psychological support in the framework of individual psychotherapy. It is necessary to manage support in the community according to each situation while clinically judging the whole "place" each time.
